Model Options Report (MOP)
A Model Options Report appraises the service needs of an individual and analyses service model options to assist the design and selection of future service supports, interventions and delivery.
It is developed with input from the person's current and past support teams and associated stakeholders.
How recommendations are formed
The report brings together the views and opinions of stakeholders alongside the reporting clinician's observations of the situation, and develops them into structured recommendations.
These recommendations are therefore not solely based on clinical formulation related to specific technical assessment by the reporting clinician.
