Behaviour Assessment Report

A Behaviour Assessment Report contains a functional analysis of key behaviours, exploring why behaviours occur and how they impact the individual and those around them.

Our clinicians review behavioural history, current presentations, environmental influences and stakeholder insights to create a comprehensive report that supports better decision-making and more effective intervention pathways.

Our assessment process

We follow a structured and clinically informed process to ensure accuracy, collaboration and quality.

  1. 01

    Contact the provider

We begin by connecting with the referring provider to gather essential background information and clarify the scope of support.

  2. 02

    Review historical and current information

Our team reviews all available documentation, behavioural history, previous interventions and current support plans.

  3. 03

    Observation and stakeholder interview

We conduct direct observations across relevant settings and interview carers, support staff and family members to build a clear understanding of the participant's needs.

  4. 04

    Travel and site-based assessment

Where required, our practitioners travel to the participant's home, school, workplace or support environment for in-person assessment.

  5. 05

    Draft report review

The report undergoes an internal quality assurance process, including:

    • Clinical Supervisor review
    • Editorial review for clarity and compliance
  6. 06

    Final report delivery

Once finalised, the Behaviour Assessment Report is sent to the provider and securely stored within our document management system.
