Functional Skills Assessment Report

A Functional Skills Assessment Report is a comprehensive report compiled from observation and interview with the client and their stakeholders.

The clinician reviews background information and current presentation to build an understanding of the person's functional capacity and everyday skills. The report is available for clients of all ages.

Tools that may be used

Tool selection is at the clinician's discretion and more than one may be used:

  • Adaptive Behaviour Assessment System (ABAS)
  • Vineland Adaptive Behaviour Scales
  • Mental health screeners
  • Assessment of Sexual Knowledge (ASK)
  • Other tools focused on functional capacity

About the tools

A Vineland assessment assesses adaptive behaviour using a combination of interviews and form responses.

An ABAS is a psychometric assessment of functional skills. It is generally completed in addition to a Functional Skills Assessment but is not mandatory.

ASK is an interview questionnaire covering sexual health knowledge, including a review of background information and a report with outcomes and recommendations.

Clinicians must hold the relevant minimum qualification to complete an ABAS, or be paired with a clinician who does.
